Contraindications
  • active bleeding, hepatic disease, pregnancy, breastfeeding, hypersensitivity, severe renal dysfunction (CrCl <30mL/min)
Drug interactions
  • ketoconazole, itraconazole, posaconazole, or ritonavir, concomitant treatment with anticoagulants
Adverse Effects
(common and severe)
  • bleeding and associated complications (weakness/paleness/dizziness/headache/dyspnea/unexplained shock)
Instructions and dose
(initial; typical)
deep vein thrombosis, pulmonary embolism treatment: 15 mg twice daily
Monitoring
  • routine monitoring of coagulation tests not required
  • complete blood count
  • renal function test prior to initiation
